Method

Application:
If you are looking for a dentist, you can apply on the home page. After we process the information in our system, we will call you to schedule an appointment.

New patient:
At your first appointment, we will ask you to complete a medical history (health questionnaire). Download medical history.

Control:
During the checkup, the entire set of teeth is examined. For any further treatments, you will need to make a new appointment. You can schedule your next checkup and/or treatment directly at the front desk.
Good thing, when you want to keep your teeth optimally maintained.

Treatment:
If you think treatment is needed, an appointment for a consultation should be made first, followed by an appointment for treatment. This way we can schedule the required treatment time.

Treatment takes place by appointment only

Appointment Making:
Appointments can be made by phone between 9:00-12:00 am & 1:00-16:00 pm or at the front desk. No appointments can be made by email.

Failure to keep appointment:
A fee will be charged for any missed appointment.

  • Cancel/reschedule appointments to < 30 min. > at least 1 business day (24 hours).
  • Cancel/reschedule appointments longer than > 30 min. > at least 2 business days (48 hours)

Based on the duration and type of treatment, costs are determined. Please remember that if you wish to cancel a long appointment on a Saturday, you must contact us by phone no later than the Wednesday afternoon before.

Pain:
If you have a very serious pain complaint, you can visit us as an emergency patient under certain conditions. In case of a very serious pain complaint outside working hours, see our emergency home page.

Payments:
As of December 1, 2019, payments will go through Infomedics to your insurer if you have dental insurance. Thus, any residual amount for which you are not insured may potentially be passed on to you by your own insurer. If you do not have dental insurance you will receive the invoice from Infomedics in the mail. Infomedics provides accounts receivable administration for medical professionals and healthcare institutions. This organization has the option for installment payments. Risk:
With crown and bridge work, among others, you will receive a warranty. However, if other dentists work on this or if one does not come for regular check-ups and does not maintain it properly, this warranty will be void.
